Arterial Thrombus in a Graves thyrotoxicosis - Hyper-coagulable state and Hyperthyroidism

Uchambally Manjima , Pernicova Ida

Hyperthyroidism is not a well-known cause of venous thromboembolism. Hyper-coagulable and hypofibrinolytic states are described in hyperthyroidism. A meta-analysis of 51 studies evaluating the consequence (exogenous and endogenous effect on coagulation, raised thyroid levels were associated with a rise in clotting factor VIII, IX, Von Willebrand factor and fibrinogen. The procoagulant effect noticed in hyperthyroidism facilitated by thyroid hormone receptor beta gene. 40-year-...

Pregnancy as a cure for a macroprolactinoma?

Koko Thet , Pernicova Ida , Ward Emma

Our patient presented at the age of 29 years with headaches and galactorrhoea and was found to have a prolactin level of more than 3000 mu/L. Her other pituitary function was normal. MRI pituitary showed 2×2 cm lesion with suprasellar extension and some distortion of the optic chiasm. She was started on Cabergoline 0.5 mg twice weekly for a presumed macroprolactinoma. Visual testing was normal. The repeat MRI four months after initiation of dopamine agonist therapy showed...

Association between rs1801260 of the circadian gene CLOCK and lipid and glucose metabolism in women with Polycystic Ovary Syndrome

Pernicova Ida , Akinci Baris , Prasai Madhu , Baxter Paul , Scott Eleanor

Introduction: The circadian clock controls numerous aspects of mammalian physiology across the 24 hour day, including lipid and glucose metabolism. Disruption of the core circadian gene CLOCK is associated with the development of obesity, diabetes and the metabolic syndrome in mice. Women with Polycystic Ovary Syndrome (PCOS) are at increased risk of developing diabetes, dyslipidemia and the metabolic syndrome. The aim of the present study was to investigate the relationship o...

Circadian clock gene BMAL1 is associated with blood pressure but not autonomic function in man

Pernicova Ida , Prasai Madhu , Maqbool Azhar , Robert West , Mary David A.S.G. , Scott Eleanor

Introduction: The circadian clock controls numerous aspects of mammalian physiology across the 24 hour day. The suprachiasmatic nucleus (SCN) is the “master clock” which coordinates the peripheral clocks in the body, including the pineal clock that receives SCN input via a multisynaptic noradrenergic pathway. Murine studies have shown a role for the core clock gene BMAL1 in the regulation of blood pressure. Selected haplotypes of BMAL1 have previously been associated...

Background: A founder mutated AIP allele, R304* was previously identified in several Irish familial isolated pituitary adenoma (FIPA) pedigrees from a small region within Mid Ulster, Northern Ireland, but the allele’s general population impact remains unknown.Aims: To estimate R304* prevalence in the general population and pituitary adenoma (PA) patients and to calculate the allele’s time to most recent common ancestor (tMRCA).<p c...
